Rachel Zimmer, RN, DNP, AGPCNP-C is an Assistant Professor of Gerontology and Geriatric Medicine at Atrium Health Wake Forest Baptist. She is a practicing adult-gerontological nurse practitioner specializing in the care of underserved populations and older adults. She is the founder and director of the Atirum Health Wake Forest Baptist Mobile health program, serving the health care needs of people who do not have health insurance. In addition, she founded a food prescription program in partnership with several community based organizations, providing locally grown food for people who have food insecurity. She utililzes community-engaged approaches in the development of care models that provide innovative solutions to addressing the health needs of people who live in marginalized communities. Her work has been funded by the USDA, Office of Rural Health, Health and Human Resources Association, and others. Her scholarly work centers around health equity, the care of older adults, and addressing food insecurity.
